Pregnancy test: when should you take it for an accurate result?

If you test early, the test may not react even if you are pregnant. That is why when you tested matters as much as the result, and a result after the expected period date has passed is more useful for judging the situation than an earlier result.

When someone seeks medical care because they are worried about pregnancy, the date is usually checked before the number of lines. A one-line result three days after sex and a one-line result one week after the expected period date convey completely different information.

This page covers only the stage before you open the test. In order, it goes through how detection works, the basis for counting the days, preparation on the day, and when to retest before pregnancy is confirmed.

01 · What the test detects

A line appears only once hCG has built up

The test looks at hormone changes after implantation, not at the day of sex.

What a urine test detects is hCG (human chorionic gonadotropin). After the fertilized egg attaches to the uterine lining, the tissue that will later become the placenta starts producing this hormone, so the reference point is after implantation, not the day of sex or the day of fertilization.

Early hCG usually rises steeply over spans of a few days, but the starting amount is very small. Each test has its own response threshold, so even if the body is already making hCG, no line forms if it is below that threshold.

  1. Sex and ovulation

Sperm can survive inside the body for several days, so fertilization may occur around a later ovulation rather than on the day of sex.

  2. Implantation

The fertilized egg usually attaches to the uterus within 1–2 weeks after ovulation, and hCG is released from then on.

  3. When the result line shows

A line forms only when hCG in the urine reaches the test's cutoff. When that happens differs from person to person and from product to product.

Even if your cycle is regular, ovulation can shift earlier or later from month to month. That is why the date of sex alone cannot tell you that a result will be certain after a set number of days.

02 · What you count the dates from

Days are counted from the expected period date rather than the day of sex

The day you test depends on whether you know your expected date.

If you know your expected period date

The starting point is a day when the expected period date has passed and there is still no period. Some products say they can be used before then, but a negative around this time needs to be checked again after the expected period date.

If the expected date is hard to estimate

If your cycle is irregular or you cannot remember the date of your last period, a common recommendation is to test at least 3 weeks (21 days) after the last time you had sex without contraception.

If it is right after sex

A test done the day after sex, or within a few days, cannot show whether that sex led to pregnancy. The result then usually only reflects whether a pregnancy that was already ongoing from before is present.

The expected period date is found by adding the number of days in your usual cycle to the first day of your last period. Many people do not have a 28-day cycle, so try calculating with your own usual interval.

03 · The day you test

Have your first morning urine and the instructions ready

Even on the same day, the result can differ depending on the condition of the urine and how the test is used.

  1. If it is still early, morning urine first

The first urine after waking has collected overnight and is concentrated, with more concentrated hCG, so many products recommend using it when testing early. Some products can be used at any time of day, so check the instructions in the package.

  2. Do not drink a lot of water before the test

After drinking a lot of water, your urine may be diluted enough for less hCG to be detected. If you plan to test during the day, it is better to wait a few hours without urinating before using the test.

  3. Follow the instructions in the exact order given

How you apply the urine, when you read the result, and the expiration date differ by product, so first read the instructions inside the package all the way through.

If the result window is unclear or the line is faint, see the separate column for how to interpret the result.

04 · Reasons for an early negative

A negative can mean 'not yet'

Even if you are pregnant, it can show as one line if the test is too early.

Still before implantation, or just after

Before implantation, hCG has not yet begun to be released, and just after implantation the amount released is small, so even a highly sensitive product may not react.

A month when ovulation was later than usual

If ovulation is delayed by a few days, implantation and the rise in hCG are delayed along with it. Even if the expected period date has passed on the calendar, by the body's own timing it can still be early.

Sensitivity differs by product

The detection threshold differs by product, so on the same day the results of two products can disagree.

A negative result early on is closer to “we do not know yet” than a finding that you are not pregnant. Using several tests on the same urine adds little information.

05 · When to test again

If pregnancy is not yet confirmed, space out the days and check again

These steps apply before pregnancy is confirmed and only if you have no abdominal pain or bleeding. If you have already seen a positive result or received a diagnosis at a hospital, seek medical care promptly instead of waiting at home and repeating tests.

  1. If you saw one line before the expected period date

If your period has not come once the expected period date has passed, retest with first morning urine.

  2. If it was still one line even after the expected period date

Ovulation may have been delayed, so retest after a few days. Follow the interval given in the instructions.

  3. If the retest is also one line, and there is still no period

By this point, the stage of repeating the test at home has passed. A blood hCG level and similar tests are used to look for the cause.

If you had sex without contraception again after your most recent test, the earlier negative result cannot tell you whether you became pregnant afterward. Start counting the days again from that date of sex.

Signs to get medical care right away, regardless of the date

  • When one side of your lower abdomen hurts or the pain is getting stronger
  • When you have dizziness, a feeling as though you may lose consciousness, or a cold sweat
  • When the bleeding is heavy, or when even a small amount continues without stopping

If pregnancy cannot be ruled out, these symptoms may point to a problem that needs prompt assessment, such as an ectopic pregnancy or miscarriage. Seek medical care without delay, regardless of how many lines appeared or whether you took a test.

If you suddenly have severe pain, or a feeling that your consciousness is fading, go to the emergency room right away or ask 119 for help.

06 · Continue by situation

This leads to guidance matched to your current situation.

If you took an emergency contraceptive pill

After an emergency contraceptive pill, the next period may come early or late, so counting the days is tricky. Guidance on when to test differs by the drug's ingredients, so I have summarized it separately in a column.

See the reference for when to test after taking the medicine

If you had a little spotting around the expected date

From the appearance of the blood alone, it is hard to tell a period from bleeding around the time of implantation. I have gathered, separately in the implantation bleeding guide, the order of checks when there is bleeding and the signs for being seen promptly.

It helps to make notes before your visit.

  • The first day of your last period, and your usual cycle (how many days apart)
  • Dates of sex you are worried about (all of them, if there was more than one)
  • The day and time of day you tested, and a photo of the result
  • Medicines taken recently (including emergency contraceptive pills and birth control pills)

07 · Questions about using a pregnancy test

Questions often asked before testing

It has been one week since sex. Can a test kit tell me?

In most cases it is too early. hCG appears only after ovulation and implantation have taken place, so a negative around this time cannot rule out pregnancy. Test again after the expected period date, or 3 weeks after sex if you do not know the expected period date.

If it is an early pregnancy test, can I use it before the expected period date too?

The first day you can use a test varies by product, so check the enclosed instructions. A positive result at this stage is meaningful, but a negative result needs to be checked again after your expected period date.

Can I do it at a time other than the morning?

Early on, first morning urine is more favorable, and some products say that if enough days have passed you may use them at any time. Follow the instructions for the product you are using.

Can a blood test at a clinic tell you sooner than a test kit?

Blood tests can measure lower amounts of hCG than urine tests, so they sometimes provide an answer a little earlier. However, if implantation has not yet occurred, a blood test cannot tell you either, so the timing of the blood draw is decided at the hospital after reviewing the dates.

What if I have abdominal pain while waiting for the retest day?

If one side of your lower abdomen hurts, you feel faint as though you might collapse, or you have heavy bleeding or bleeding that does not stop, come to the hospital immediately instead of waiting for the retest date. If symptoms worsen rapidly, go to the emergency department first.
